    Medications at School (All Students)

    Please note that all students of Currituck County Schools will need a new Permission to Administer Medication form completed by your healthcare provider before the school nurse can give any medication during school. The form has to be signed by the provider and the parent. This includes both prescription and over-the-counter medications like Tylenol, Ibuprofen, etc.  The medication needs to be in the original bottle or box with the prescription label from the pharmacy and any over-the counter medications need to have the student’s name written on the package, with the proper ordered dosing according to the providers’ orders on the package. Please note that all medication forms are vaild for 1 school year.

    Medications at School (PreK Students only)

    Prek students (before kindergarten) of Currituck County Schools have an additional Medication Administration Permission form that is completed by parents/guardians before the school nurse can give any medication during school. The form has to be signed by a parent/guardian. This includes both prescription and over-the-counter medications like Tylenol, Ibuprofen, etc.  The medication needs to be in the original bottle or box with the prescription label from the pharmacy and any over-the counter medications need to have the student’s name written on the package, with the proper ordered dosing according to the providers’ orders on the package. Please note that all PreK medication forms are vaild for 6 months.

    Asthma Medications

    If your student has asthma and requires an inhaler at school, then you will also need an Asthma Action Plan form completed by your provider.

    Allergy Medications

    If your student has a life-threatening allergy and requires medication at school (such as Benadryl, Epi Pen, etc.) then you will also need an Allergy Action Plan form completed by your provider.

    If your student has an allergy to a food (such as peanuts, tree nuts, milk products, etc.) then you will also need a nutritional needs form completed by your provider.

    Please keep in mind this is a yearly process. Orders and medication forms from the last school year are not kept. The first day of school begins a new year for students as well as nursing records.  Let's have a safe and healthy school year!
