Parent/Guardian information

• Photo ID is a required upload

Student information: Dates should be entered as MM/DD/YYY

• Demographic
• Health/medication information
• Birth certificate is a required upload
• Immunization (You have 30 days from the first day of attendance to bring this to the school if you can’t upload it with this application)
• For students new to NC - NC Health Assessment (You have 30 days from the first day of attendance to bring this to the school if you can’t upload it with this application)
• Notary Form (Can be uploaded or delivered to the school)
• 504 Plan (Can be uploaded or delivered to the school)
• IEP (Can be uploaded or delivered to the school)
• AIG (Can be uploaded or delivered to the school)
• EL Plan (Can be uploaded or delivered to the school)

Emergency Contact (people in addition to parents/guardians)
