    Listed below are links to health and safety forms that need to be completed by ALL participating student-athletes. 

    Student-Athletes must have a current physical on file with the Athletic Trainer before they can participate in any sport.  If the physical expires during the student-athletes sports season, the student-athlete will not be allowed to participate until an updated physical has been given to the Athletic Trainer. If you are unsure about when the physical expires, please contact Halie Sweeney our Athletic Trainer.

     

    2023-24 Athletic Physical Form

     

    Student-Athletes must complete the Currituck County High School's Athletic Packet before they can tryout/practice for a sport. This packet contains vital information that is required by the North Carolina High School Athletic Association and by Currituck County High School. As of July 2019, these packets will be completed online through the RANK ONE  computer program.  Please attend the PRE-SEASON MEETINGS to get the information on how this is done.

     

     RANK ONE PAPERWORK

     

    Currituck County High School has a Drug Testing Program.  The drug testing is random and it involves any student-athlete and other students at the high school who participate in extra-curricular activities. All student-athletes must opt-in to the testing program. Student-athletes will receive this form once team rosters have been made.  This must be returned to the school before the student-athlete can participate with their sport.
