Forms & Requirements

    • NC Physicals are required for students who are entering Kindergarten or new to NC Schools. 

     

    • Immunizations are required at Kindergarten, 7th grade, and 12th grade and must be done by the 30th day of school. K-12 School Requirements

     

    • Health Cards - Please return the health cards ASAP!  It is helpful for the nurse to have this information in order to provide the best care for your child.  If your child has a health condition, allergy or medication, there are specific forms that must be completed.  These forms are located on the CCS website.

     

    • Permission to Administer Medication This form must be completed by your pediatrician before the school can give any medication.  Medication must be in the original bottle or box with the RX label, and any OTC medications need to have the student’s name clearly printed on the container, along with the proper doctor’s orders on the package (this includes Tylenol, Ibuprofen, etc.). Medication changes require a new doctor’s order.  Please bring a note from the doctor.  Parents must pick up the old medication if the medication in the nurse’s office has been discontinued.
      Keep in mind this is a yearly process.  Orders or medications from the previous school year are NOT kept.  The first day of school begins a new year for nursing records. Parents/Guardians are encouraged to contact the school nurse if you have any questions regarding the administration of medication at the school.

    • Each school provides diagnostic COVID testing for students and staff.  Please contact the school nurse to sign up.
