Health Services Offered

  • Students must be healthy to be educated and educated to be healthy!

     

    Services That Require Prior Written Consent:

    • Medication Administration:  We must have a Permission to Administer Medication form on file with physician and parent signatures.  All medications must be brought in by an adult to the school nurse in the original prescription (or non-prescription) bottle (tube).  If the medication is for a specific condition (ie: asthma, seizures, diabetes, etc.) a care plan may be mandatory as well.   
    • Healthcare Procedures that require written authorization from a healthcare provider along with parental consent (tube feedings, diabetes care, catheterizations, etc).
    • Individual Vision/Hearing Screenings
    • Health Screenings for Exceptional Children's Evaluations
    • Dental Screenings
    • Puberty Education

     Services That Do Not Require Prior Written Consent:

    • Care for students with illness/injury
    • Basic health education and counseling
    • Follow up on injuries 
    • Immunization/ Physical compliance
    • Communicable disease surveillance

    Services That Can be "Opted Out":

    • Mass Vision (Grades 1, 3, and 6)
