Immunization and Physical Requirements for Kindergarten and/or First Time Enrollment in NC School Systems
Per N.C.General Statute 130A-152-157:
To be enrolled in Kindergarten or above in a NC Public School your student must have the required vaccines to include:
5 DTP/DTaP/DT
4 Polio4 Hib3 Hepatitis B2 MMR2 Varicella4 PneumoccalThere are only 2 exemptions recognized by N.C. and they are religious and medical. Please speak to your school nurse if these apply.In addition to your student being current on vaccines it is also required to have a completed N.C. Health Assessment Form. This assessment (completed by your physician) must be less than 12 months old at the time of entry. See N.C. General Statute 130A-440; 10A NCAC09.3005. There is only one exemption recognized by N.C. for the assessment and that is a religious exemption. Please see your school nurse if this applies to your student.The proof of current vaccines and the N.C. Health Assessment Form must be turned into your school nurse's office by the 30th day of enrollment.
